Clinical scope

Three modules, each with its own question.

The tumor profile, hereditary risk and drug response are answered by different analyses and presented in separate parts of the report, each with its methodology and limitations.

01 / Tumor

Somatic profile

NeoPainel Somático analyzes sequence variants, copy number and DNA fusions in 560 cancer-associated genes, plus TMB and MSI, in tumor tissue.

See the 560 genes
02 / Heredity

Hereditary cancer predisposition

The Super Painel Cancer germline module assesses 106 genes associated with hereditary cancer syndromes, from whole-genome sequencing.

03 / Medications

Pharmacogenomics in oncology

CYP2C19, CYP2D6, DPYD, NUDT15, TPMT and UGT1A1, with recommendations for anticancer drugs and supportive care medications.

Integrated view

Why bring the three modules together.

The result should be interpreted by the treating physician together with clinical and histopathological data. Therapy selection depends on tumor type, current regulatory approvals and clinical judgment.

  1. 01

    The tumor guides therapy

Somatic alterations, TMB and MSI classified by clinical relevance to support decisions on targeted therapies, immunotherapy and clinical trials.

  2. 02

    Heredity changes family care

Without a normal sample, a variant in the tumor may be germline. The germline module, performed on a non-tumor sample, directly assesses the hereditary predisposition genes.

  3. 03

    Medications in the same context

Genes linked to the metabolism of fluoropyrimidines, irinotecan, thiopurines and tamoxifen, and of supportive medications such as antiemetics, analgesics and antidepressants.

Scope and limits

Two samples, explicit limits.

Two samples: tumor tissue for the somatic profile and a germline sample for the heredity and pharmacogenomics modules.

The analysis considers

Tumor, germline and medications.

  • Tumor: paraffin block (FFPE), with the tumor area selected by the pathologist, or DNA extracted from the tumor.
  • Germline sample: peripheral blood, buccal swab or extracted DNA.
  • Anticancer drugs assessed: fluorouracil, capecitabine, irinotecan, mercaptopurine, thioguanine and tamoxifen.
  • Supportive care: ondansetron, tropisetron, codeine, amitriptyline, nortriptyline and paroxetine.

Important limitations

Each module has its own limits.

  • Somatic profile: analysis restricted to the regions captured by the panel; variants at low allele frequency or in samples with low tumor fraction may not be identified.
  • Germline module: repetitive, high-homology or insufficiently covered regions may not be adequately analyzed.

Choice of test

NeoPainel Somático or Neo360?

Both analyze the tumor with the same 560-gene panel. Neo360 adds hereditary cancer predisposition and pharmacogenomics in an integrated report.

Why germline analysis matters: without a matched normal sample, a variant found in the tumor may be germline. Variants with an allele frequency near 50% or 100% in hereditary predisposition genes should be confirmed in a non-tumor sample, with genetic counseling. Neo360 includes this analysis for 106 hereditary cancer genes.

Follow-up after your results

Infinity VUS

All NeoGenomica tests include Infinity VUS to follow reported variants of uncertain significance. When a relevant reclassification is confirmed after specialist review, the doctor is notified and the report may be updated.

Testing process

From two samples to an integrated report.

One order coordinates the analysis of the tumor and the germline sample.

  1. 01

    Medical order

Tumor type, clinical data, family history and current medications guide the analysis.

  2. 02

    Tumor sample

Paraffin block with the tumor area selected by the pathologist, or DNA extracted from the tumor.

  3. 03

    Germline sample

Peripheral blood, buccal swab or extracted DNA for the heredity and pharmacogenomics modules.

  4. 04

    Analyses

Somatic panel on the tumor and whole genome on the germline sample.

  5. 05

    Integrated report

Three parts, each with its findings, methodology and limitations.

Frequently asked questions

Before ordering Neo360.

What is the difference between Neo360 and NeoPainel Somático?

NeoPainel Somático analyzes only the tumor. Neo360 includes the same somatic profile and adds hereditary cancer predisposition, across 106 genes, and pharmacogenomics in an integrated report.

Which samples are needed?

Two samples: a paraffin block (FFPE) with tumor tissue, or DNA extracted from the tumor, for the somatic profile; and peripheral blood, a buccal swab or extracted DNA for the germline and pharmacogenomics modules.

Is the germline result relevant to the family?

It may be. Germline variants may be present in relatives, and the report includes information relevant to them. Discussion with genetic counseling is recommended when there is a germline finding.

Does pharmacogenomics define the treatment dose?

Not on its own. The result complements clinical assessment, and cancer medications should not be changed based on the test alone.

When will the report be ready?

15 calendar days, after acceptance of the sample .

Does the test require a medical request?

Yes. The tumor type, clinical and histopathological data, family history and current medications guide the analysis and interpretation.

Technical details for Neo360

Method, scope, requirements and limitations

Integrated precision oncology: molecular tumor profile, hereditary cancer predisposition and pharmacogenomics in a single report.

Applications
Hereditary cancer, Precision oncology, Pharmacogenomics
Accepted samples
Peripheral blood (preferred), Buccal swab, Tumor tissue in paraffin (FFPE) (preferred), Extracted DNA
Estimated turnaround time
15 calendar days, after acceptance of the sample
Methodology
Somatic module: next-generation sequencing (NGS) of DNA with hybrid capture using the Twist Oncology DNA CGP panel and bioinformatic analysis with the NeoSoma pipeline, identifying and annotating SNVs, small insertions and deletions, copy number alterations, rearrangements and fusions predicted in DNA, TMB and MSI. Germline module: whole-genome sequencing (WGS) with a curated panel of 106 risk genes. Pharmacogenomics: analysis from the whole-genome data, with variant interpretation using the PyPGx tool, PharmGKB and CPIC guidelines.
Analytical scope
Somatic profile: 560 cancer-associated genes, with SNVs, indels and multinucleotide variants, copy number alterations, DNA fusions and rearrangements, TMB and MSI. Hereditary predisposition: 106 genes associated with hereditary cancer syndromes. Pharmacogenomics: CYP2C19, CYP2D6, DPYD, NUDT15, TPMT and UGT1A1, with recommendations for 12 drugs across anticancer therapy and supportive care.
Medical order
Required

Requirements

Medical order with the tumor type, clinical and histopathological information, personal and family history and current medications. Two samples: a paraffin block (FFPE) with tumor tissue, with the tumor area selected by the pathologist, or DNA extracted from the tumor, for the somatic module; and peripheral blood, a buccal swab or extracted DNA for the germline and pharmacogenomics modules.

Limitations

Somatic module: analysis restricted to the regions captured by the panel. Variants at low allele frequency, below the limit of detection or in samples with low tumor fraction may not be identified. Fusions and rearrangements are assessed in DNA, only in the captured intronic regions. TMB and MSI are panel-based estimates and are not directly interchangeable with those of other methods. Germline module: repetitive, high-homology or insufficiently covered or low-quality regions may not be adequately sequenced or analyzed. Pharmacogenomics: metabolizer phenotypes are inferences based on known variants and may not capture all genetic or environmental influences or drug interactions.

Additional information

Each module is presented in its own part of the report, with methodology and limitations. Missing data for a module is not equivalent to a negative result. Cancer medications should not be changed based on pharmacogenomics alone. Infinity VUS is included for variants of uncertain significance reported in the germline module.
